This notice describes information provided through Revive Dental’s website and review-request texting program. It does not replace the practice’s notice of privacy practices for patient health records. Please do not submit medical details through the texting opt-in form.
Information you provide
When you choose to receive review-request texts, we collect your name, mobile number, consent choice, the wording you agreed to, the source page, and submission time. We use this information to record your preference and administer the program. Website systems also process technical request information to deliver forms and limit abuse.
How information is used and shared
We use service providers, including Advanced Web Systems and messaging providers, to process and securely store submissions and deliver the services you request. Mobile information will not be shared with third parties or affiliates for marketing or promotional purposes. Text messaging opt-in data and consent will not be sold or shared with third parties, except service providers necessary to operate the text messaging program.
Your choices
Texting consent is optional and is not required to receive care or make a purchase. Reply STOP to a program text to unsubscribe, or call (318) 878-2405 to update or withdraw your preference. Reply HELP for messaging help. We retain preference records as needed to administer the program and document consent or withdrawal. Contact the office with questions about your information.
